Registered Nurse (RN) Description

Responsible for planning, developing, organizing, implementing, and directing care programs for all residents.


  • Provide health care needs specific to each resident’s records, follow correct procedure and complete all required documentation to ensure delivery of care:
  • Coordinate and implement the delivery of resident care
  • Complete assessments and write service plans for current and prospective residents
  • Responsible for maintaining electronic health record
  • Responsible for notifying doctor and family of any health concerns
  • Responsible for documenting resident’s health concerns
  • Uphold confidentiality and Health Insurance and Portability and Accountability Act (HIPAA) regulations
  • Assists with medication management
  • Monitor medications for all residents who receive medication management every 30 days
  • Ensure med documentation and tasks are completed each shift
  • Assure appropriate medical information is relayed between shifts
  • Train new and existing employees
  • Conduct health care related portion of the new hire orientation and onboarding
  • Train each new and existing employee, thoroughly, in his/her duties and in the policies and procedures as they relate to their position
  • Participate in the safety team:
  • Be a part of the safety team and committees as assigned
  • Use and train on proper body mechanics
  • Be responsible for proper disposal of hazardous materials
  • Train on the use of personal protective equipment, as required, and monitor staff for appropriate usage
  • Review and follow up on incident reports. Ensure incidents are documented and entered into electronic health record.
  • Schedules teams for adequate and appropriate care
  • Supports community Director as designated manager on duty and “on call” designee for after hours and weekends

Required Skills & Abilities

  • Two years’ experience in working with persons with dementia including leading groups, creating social interactions, assistance with ADLs.
  • Experience assessing resident needs and developing approaches to reduce discomfort/enhance daily well-being
  • Must be able to relate professionally and positively to community staff
  • Must meet state regulatory requirements for the position.
  • Experience working with seniors is preferred.

Education & Experience Requirements

  • RN – Registered Nurse, current state of employment
  • CPR Certification
  • Must be able to work irregular hours, weekends and evenings.

Physical Requirements

  • Ability to drive and travel to various sites as needed.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Prolonged periods of standing and moving.
  • Must be able to lift up to 50 pounds at times with or without assistance.
